How much does the software cost?
You only pay for CCM360 when you use it to deliver chronic care management (CCM) services to one of your enrolled beneficiaries.  We call this a transaction.  Rather than paying an annual license, we charge a $4 fee for each completed transaction.  On average, this is 9.2%  of what you bill Medicare for the CCM services.  We track your transactions and generate a detailed report.  You are invoiced at the end of the calendar quarter.

What constitutes a transaction?
A transaction occurs when you use the software to perform a medical history and generate results, such as follow-up recommendations, objective scores, care plan, educational material, the CCM note, and the hyper link to the patient's virtual health coach.  In most cases, each enrolled patient will generate one transaction per month. One hundred patients who receive CCM services from your office will only cost $400 per month in transaction fees.

How do I achieve the required 20 minutes of non-face-to-face clinical time per patient?
Ideally, your MA will spend 10-15 minutes performing a monthly check-in using CCM360.  This includes collecting data and generating the CCM note.  You (the PCP) will provide the remaining 5-10 minutes of required time when you review and sign-off on the CCM note.  A reasonable target is splitting the time evenly between the MA and the PCP.

Are there any setup costs?

How much does the training cost?
Nothing.  We offer comprehensive web-based training by a registered nurse every month.  We can get your staff competent on the system in one hour.

Which code am I billing?
CPT code 99490 is a new code for non-face-to-face chronic care management services.  To meet the requirement of the code you must perform at least 20 minutes of non-face-to-face services (by physician or another health professional) per month.

Are there any other requirements for billing 99490?
Yes.  We recommend you review the references listed at the bottom of this page as well as consulting the 2015 Medicare Physician Fee Schedule.

Is the virtual health coach included in the price?
Yes.  We created this feature so the patient could learn at their own pace and convenience.  The virtual health coach also contains the electronic care plan in an interactive format to speed learning and self-improvement.  The virtual health coach also allows the patient to re-evaluate themselves by pressing the Check Progress button.  Any evaluations that are performed by the patient are not treated as billable transactions.

Can our practice place its logo on the virtual health coach?
Yes, and this service is free.  You will need to provide us with a graphic in an appropriate format (JPEG, PNG, etc.), we can place this logo at the bottom of the page.  We can also make this logo link to the web page for your practice. 

Clinical Questions:

Will using the CCM360 software change physician workflow?
No.  We anticipate the following workflow:

  1. Medical assistant calls the patient once a month to perform the interview, generate the care plan, educate the patient, generate a virtual health coach, and produce a care management note.
  2. Medical assistant sends the patient a link to their own virtual health coach.  It contains an electronic version of the care plan and educational content.
  3. Medical assistant sends care management note to PCP and-or saves it as a PDF that can be appended to the office EMR.
  4. PCP reviews the care management note.

Who trains my clinical staff to use CCM360?
DSHI Systems clinical staff (an RN) provides web-based training to your medical assistants, nurses, and other medical staff.  Our training course was produced by a team of physicians and nurses. DSHI offers free training sessions every month.  Perfect for new staff or as a refresher course for existing staff.

Who wrote the content in CCM360?
CCM360 content is written and maintained by board certified physicians at DSHI Systems.  Content is evidence based and peer reviewed. Contact DSHI to obtain a bibliography for the content.

Yes.  More than a dozen new clinical workflows are in development.  All new workflows are available to the licensed user at no additional cost.

How often is the content updated?
Clinical content is updated quarterly.

Technical Questions:

Do I have to install any software?
No.  CCM360 is a web service that is accessed using a browser. The software resides on CCM360 servers in a secure data center in Nashville, TN.

Are there any software requirements?
Yes.  To use CCM360 in the office, you must have Internet Explorer V9 or above.  The virtual health coach (provided to the patient) has less restrictive requirements and can be used on IE, Firefox, Safari, and Chrome.  It also displays nicely on iOS and Android devices.

How do I get the care management note into my EMR?
CCM360 allows you to save the CCM note as a PDF, that can be appended to any EMR.

Do you plan to integrate CCM360 with any commercial EMRs?
Yes.  Discussions are in progress to integrate CCM360 into some of the more popular physician practice EMRs.  CCM360 is also available as a widget for easy integration into an EMR or CRM application.  Please contact DSHI for details.

Is the site or service HIPAA compliant?
The CCM360 server is not subject to HIPAA certification since it does not receive, send, or store personal health information.  All transactions are performed anonymously.

Is the site or service secure?
The CCM360 service is hosted at a HIPPA compliant data center.  The CCM360 domain uses Hyper Text Transfer Protocol with Secure Sockets Layer (https).  Hyper Text Transfer Protocol Secure (HTTPS) is the secure version of HTTP, the protocol over which data is sent between your browser and the website that you are connected to. It means all communications between your browser and the website are encrypted.

Does the site store usage data?
No.  A future update will allow CCM360 customers to download their usage data (tab-delimited) at anytime for parsing into a database. Customers who are using the CCM360 widget have the option to obtain JSON data at the close of each encounter.  This data can be used to build a data repository to power population health and predictive analytics.  Please contact DSHI for details.
